I was informed:
1. that providing my personal data is voluntary, however in the event of failure to do so, I will not be able to take part in the New Product Development Training “NPD Skills”,
2. about the right to demand the amendment, deletion, or limitation of the processing of personal data and the exercise of the right to object to such processing as well as the right to withdraw my consent at any time by sending an e-mail with a request to the following address: npdskills@wz.uw.edu.pl,
3. that my personal data may be shared with other participants of the Event, EIT Food izvw, EIT Food, CLC North-East, CLC South, with its registered seat in Madrid, Serrano Anguita 13, and the University of Warsaw, Krakowskie Przedmieście 26/28, 00-927 Warszawa, in order to report the Event.
4. that the Controller of my personal data is the University of Warsaw, Krakowskie Przedmieście 26/28,
00-927 Warszawa.
5. that my personal data will be stored until the processing of my personal data is no longer necessary for the intended purpose or my consent to the processing of data is revoked by me,
6. that the Controller do not intend to transfer my contact data to a third country or international organization,
7. that my personal data is not subject of automated decision-making, including profiling,
8. that I have the right to lodge a complaint with the relevant and competent supervisory authority of my habitual residence, place of work, or place of infringement if I decide that the processing of my personal data violates the regulations of the General Data Protection Regulation.
I hereby give my consent to the processing of my personal data contained in this Participant Consent Form strictly for the purposes of my participation in the Activity. This consent is given in accordance with the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons regarding the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).
Also, I hereby give my consent to the University of Warsaw and other third parties engaged in the activity to use my image and/or testimonial recorded during the New Product Development Training “NPD Skills” to be used free of charge for promotional purposes of the course, including but not limited to print and online media, the University of Warsaw and the EIT Food publications, websites, and online multimedia platforms. This includes the use of image, video, voice, or all three of them for an unlimited period. I further understand that these items may be subject to reasonable modification or editing.
